Error management in issue 13 of The Navigator

Everyone makes mistakes. Onboard ship the smallest mistake can have catastrophic effects. The latest issue of the free magazine The Navigator aims to teach its readers how to identify, avoid and capture errors. Articles discuss how technology can affect error management and the importance of teamwork and proactive and reactive techniques.

Emma Ward, Editor of The Navigator, said: "We come across minor errors and mistakes in every aspect of our daily lives and usually, these can be corrected without too much of an impact. However, when faced with navigating a ship and protecting its crew, passengers, cargo and wider environment, it is important to try and avoid errors. We hope this issue of The Navigator will give readers some guidance in how to manage their own workloads to minimise the chances of something going wrong.”

David Patraiko, Director of Projects for The Nautical Institute, added: "Many factors can increase the likelihood of a mistake, such as fatigue, stress, distraction, multi-tasking, poor visibility, complacency, heavy traffic or navigational hazards. Good passage planning is essential to identifying where errors are more likely to occur, as is establishing a ‘just culture’ on board that will explore how and why an error may have happened and to learn from it. Navigators should identify and share near-misses, and should also be encouraged to ‘Call the Master’ in times of doubt.”
