06/027 - Mooring fatigue incident
On a chemical tanker running on very short fixed run of about 2-3 days, there was a total of 6 deck crew. Out of which 4 were AB seamen doing 6 hrs on and off while at port and prior arrival of port, preparing the tanks ready for inspection.
The incident happened while carting off from a berth. One of the AB’s whose watch was just finished before unberthing, that is, after doing a watch of 6 hrs, about 20 minutes later the stations were called.
While unberting the AB was doing the aft station and while he was picking up the rope with the winch he had entangled his hand between the rope and the deadman roller.
