93052 Person Injury Near Miss Drum Fell

The crew were cleaning #1 cargo hold. The Bosun asked an AB to lower an empty drum into the hold for the waste. The AB tied a heaving line to the sling of the drum and lowered it down. Suddenly, the heaving line got loose and the drum fell on to the tank top. The Bosun was standing one meter away from where the drum fell. The Chief Officer investigated the incident and found that the heaving line had not been properly made fast to the sling attached to the drum.
