Sichem Osprey

10 Feb 2010 Resource

Navigating using paper charts, with the passage being monitored on ECS. No cross-checking of planned route.

The captain, the first officer and the third officer had, de facto, validated the course shaped and the radar adjustments done by the navigation officer. This lack of  cohesion between officers who do not practice « cross control » of the tasks in order to detect errors or omissions is an underlying factor strongly contributing to the  accident

Assessments on the familiarization with the equipment are carried out before joining the vessel, but it seems that SICHEM OSPREY officers would not have been assessed before commencing the voyage. This lack of assessment and of training, chargeable to the vessel technical manager, could also be an underlying factor of the accident.
