Casualties - fatigue factor

May 2016 - m.v. Seven Seas grounding off Larnaca.
Immediate cause
The vessel grounded as a result of the Master falling asleep / a short "micro-sleep" during which the progress of the vessel was not being monitored therefore, not anchored in time.
Contributing cause
Fatigue is considered a factor to the accident.
