Submit a Report

We want to hear about any unsafe practices, dangerous occurrences, accidents, near-miss situations or equipment failures which you have experienced, and any practices / methods you have adopted to prevent a recurrence. Within the context of ship operations, the scheme is unlimited, international in outlook, and open to commercial, naval, fishing and pleasure users.

Your participation in MARS is also evidence of your commitment to the highest ideals of quality management and social responsibility, as sought through schemes such as the ISO Standards, the ISM Code, and the Tanker Management Self Assessment (TMSA) initiative.

All reports received are treated in the strictest confidence and stored temporarily on the Institute’s server for back-up purposes until published in SEAWAYS and uploaded to the website. Reports in which the confidentiality of the identity of vessel, company, port, terminal or individual(s) may be compromised even after careful editing, will not be published. Any original written report will be destroyed prior to publication or, if not published, sent back to the contributor or sender.

A ‘Letter of Appreciation’ is also sent to contributors. For ship operators, such contributions show the management’s willingness to share lessons with the global maritime industry to improve operational, environmental and commercial safety, and achieve continuing professional development. It can also be presented as evidence during audits of the organisation’s commitment to the highest ideals of quality management and social responsibility, as envisaged by ISO Standards, the ISM Code, and the Tanker Management Self Assessment (TMSA) initiative.
